CURRENTLY HIRING:
RN ADMISSIONS NURSE – FT
The Admissions Nurse serves as liaison to and conduit between the Administrator, the patients and their families, and the community. Ensures that only those patients for whom the hospice can provide adequate care are admitted to the hospice service. The Admissions Nurse carries out the organization’s purposes in a manner consistent with the goals, objectives, and established policies.
QUALIFICATIONS:
Education: Graduate of an accredited school of nursing with bachelor or higher degree in Nursing or another health-related field. Equivalent job experience may be substituted as long as the candidate is a licensed registered nurse.
Credentials: Current, active RN license within the State. Certification in Hospice and Palliative Nursing desired.
Experience: Minimum of one-year experience as a professional nurse within the last 3 years or have a Baccalaureate in Nursing from a program accredited by the National League for Nursing.
Core Competencies: Demonstrated competency in performance of professional nursing functions. The nurse must possess the appropriate knowledge of applicable laws, regulations and standards. Excellent communication skills are required. The ability to organize, delegate, supervise and evaluate staff and understand the roles and responsibilities of the hospice interdisciplinary team is critical. Must possess a sympathetic attitude toward caring for the hospice patient and demonstrate positive communication skills in interacting with other members of the hospice team.

FUNCTIONS & RESPONSIBILITIES
- Performs comprehensive nursing evaluations.
- Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action.
- Initiates plan of care and appropriate services, provides proper nursing follow- up to all patients. Instructs caregivers in managing patient needs and submits suitable documentation within 24 hours of visit.
- Coordinates all services provided to patients/families in all settings to ensure quality of care and services as well as appropriateness of care.
- Supervises the implementation of the approved plan of care, modifies as needed, and contacts physician when appropriate.
- Provides supervision of Hospice Aides and Licensed Vocational Nurses.
- Maintains knowledge of state and federal regulations.
- Participates in QAPI program.
- Performs safety inspections identifying safety hazards in the patient’s and organization’s environments taking appropriate steps to reduce or eliminate them.
- Adheres to the highest standards of professional and personal conduct in the performance of his/her duties.
- Maintains continuing education through academic studies, seminars, and workshops.
- Attends and participates in staff meetings, in-services and patient care conferences.
- Completes all documentation in an accurate and legible manner.
- Documents aptly to the diagnosis, clearly and concisely with supportive evidence and proper terminology.
- Complies with hospice policies and procedures; laws and regulations; and standards of practice.
- Works on-call as may be needed.
Admissions Process
- In conjunction with the DPCS is responsible for overall compliance with applicable laws, regulations and standards related to patient care.
- Responsible for the oversight of the assessment and evaluation, intervention and documentation of patient and family needs as pertains to the admission process.
- Maintains an open line of communication with the DPCS and Administrator.
- Coordinates efforts with the implementation of the Quality Assessment and Performance Improvement program relating to Admissions.
- Acts as a patient advocate.
- Educates patient and family with a thorough explanation of benefits as requested.
- Assures that all clinical decision-making follows ethical guidelines.
- Maintains up-to-date resource information to facilitate independent on-going education for maintaining high-level quality of care for hospice patients & families.
- Initiates plan of care and completes admission paperwork.
- Coordinates all services provided to patients/families in all settings to ensure quality of care and services as well as suitability of care.
- Demonstrates flexibility in scheduling, and readily accepts new admissions and emergency assignments as requested.
- Educates the community in regards to hospice and acts as a liaison between the community and management.
- Assists with the mentoring of new employees to the hospice Philosophy.
- Performs additional duties as requested by supervisor.
RN CASE MANAGER – FT
The Hospice RN is an experienced registered professional nurse who initiates and coordinates the hospice care plan, provides skilled and palliative care and coordinates the services provided to his/her patients. The RN works cooperatively with the attending physician, Hospice Medical Director, and other members of the interdisciplinary Hospice team. The RN participates in patient care conferences and supervises the Hospice Aides assigned. The RN provides care, as described in this position description, to patients who have been diagnosed with a terminal illness. The RN regularly reviews and updates the plan of care. Performs prescribed medical treatments, including pain management and symptom control, conducts assessments and evaluations, provides education and supportive care to patient and caregiver.

LVN ON-CALL
Under the direction of a Registered Nurse, the Hospice Licensed Vocational Nurse provides skilled nursing and palliative care for assigned hospice patients. He/she works cooperatively with the other members of the Interdisciplinary Hospice Team. The Hospice LVN provides intermittent visits or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary care giver, and participates in the care plan development and implementation.
QUALIFICATIONS:
Education: Graduate of an accredited school of vocational/practical nursing.
Credentials: Current license within the State. Certification in Hospice and Palliative Vocational/Practical Nursing.
Experience: At least one year experience as a LVN/LPN is required.
Core Competencies: Demonstrated competency in performance of skilled nursing functions. Ability to work with dying patients and their families. Possesses a sympathetic attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team.

- Reports changes in the patient’s conditions and needs to the RN Case Manager or Director of Patient Care Services.
- Drives to and from patient’s residence to administer prescribed treatments and medications as permitted by scope of practice.

- Educates and instructs the patient and patient’s family.
- Demonstrates flexibility in patient scheduling and readily accepts new assignments.
