Our service philosophy reflects a team approach dedicated to excellence in all aspects of the patient care delivery system. We take into consideration every factor that is involved in the patients’ physical, psychosocial, emotional and spiritual well-being. Our comprehensive, interdisciplinary program of care is designed to support patient’s choices, preserve dignity, and allow the opportunity to live life as fully as possible in the comfort of their home or other chosen setting like a residential care facility or nursing home.
Hospice patients are cared for by an interdisciplinary team (IDT) composed of professionals who are specially trained to address every aspect of end- of-life care. The team meets on a regular basis with the patient and family (as necessary and when able), as well as with each other to update and carry out the hospice plan-of-care.
The Interdisciplinary Team:
The Patient/Family – The patient and their family’s (if able) participation is crucial in the development of a personalized, comprehensive and effective plan-of-care.
Primary Care Physician – The patient has the option to keep his/her primary care physician. He/she maintains primary medical care and works closely with the hospice staff in developing a personalized care plan.
Medical Director(s)/Physicians – They are board certified in hospice and palliative care. They consult with the members of the team including the patient’s primary care physician. They can also serve as the patient’s primary care physician. Our physicians make home visits as needed.
Registered Nurses/Licensed Vocational Nurses – They serve as case managers and visit the patient as often as needed, assessing physical needs and providing overall care. Our nurses educate family members and/or caregivers about the disease process and provide expert pain and symptom control. Our nurses are available 24 hours a day, every day.
Medical Social Workers – They assess the overall emotional, psycho-social spiritual and financial needs of the patient and family. They also serve as a resource for community services and assist in determining the right course of action to help the patient receive maximum support and care.
Certified Hospice Aides – They give patients professional personal care and attention. Some of their services include bathing, grooming, changing bed linens, assisting with their activities of daily living and other light household tasks.
Registered Dietician – When a patient’s nutritional intake becomes an issue, our dietician makes an assessment and provides alternative methods to ensure adequate nourishment and satisfaction.
Chaplains – We respect all religious beliefs and cultures. Our chaplains are non-denominational and provide spiritual and pastoral support for patients and families if they wish. Also, they communicate with patient’s clergy or faith-based community resources when requested.
Bereavement Coordinator – Support to bereaved families is available for up to 13 months following the death of their loved one. It comes in the form of phone calls, visits, mailings, support groups, volunteer contacts, special events and annual memorial services. These activities allow them various opportunities to share their feelings as they journey through the healing process.
Pharmacists – Although the participation of pharmacists is not a mandated IDT requirement, there are instances when we consult with them so that our patients are assured of the most appropriate use of medications to provide comfort with minimal side effects. They are available to speak directly with patients and families to educate and alleviate concerns over medications and emergency kits kept in the home.
Volunteers – Our volunteers receive specialized training and are well-prepared to offer respite care, companionship and emotional support and take care of errands. Family members also appreciate a visit from a compassionate friend who listens and understands what they are going through.
The services of a Physical Therapist, Occupational Therapist and Speech Therapist are provided as needed according to the plan of care.
